If you would like to register for any or all of our workshops, offered at these listed workshop locations, please use one of the following three methods:
- Print and complete the Registration Form
 and send by fax or mail.
- Authorize and confirm your registation by telephone.

Cancellation Policy
If you need to cancel your registration, your money will be returned minus a 10% handling fee. Cancellations received less than two weeks prior to the date of your workshop will be refunded minus 50%. No refunds will be given for cancellations or no shows the day of the workshop. Substitutions are permitted.
If sufficient registration is not received, PRIVA-CTM reserves the right to reschedule or cancel the workshop(s). In the event of cancellation, participants will receive a full refund for registration fees only.
